Secure-Video (Telehealth) Consent

1. Secure-video sessions

Some sessions may be held by secure video using Doxy.me, a private platform chosen for confidentiality. This consent applies whenever care is provided by video. Video sessions carry the same confidentiality commitments, and the same limits, as in-person sessions, as described in the Confidentiality & Privacy Notice.

4. Not for emergencies

Video sessions are not an emergency service and are not monitored at all hours. In a crisis, or if you or someone else may be in danger, call or text 988 or call 911, or go to your nearest emergency room. Do not rely on a video link to reach help in an emergency.

5. Withdrawing consent

You may withdraw this consent to video sessions at any time, without affecting your care, by telling your minister. Withdrawing consent applies to future sessions.

6. Electronic signature and acknowledgment

You agree that completing this consent online and typing your name constitutes your legal electronic signature, with the same effect as a handwritten signature. By signing, I confirm that I have read and understood this Secure-Video (Telehealth) Consent and that I consent to receive care by secure video under these terms.
